BPLog.ca is an independent tool for keeping a blood pressure care plan and home readings together.
A companion to your appointments
The two-page handout gives you space for the plan agreed with your healthcare provider, medication changes, your next appointment and a record of home readings. Download a fillable copy or print a blank one.
An early version was distributed in clinic during a Family Medicine rotation in early 2026. The current handout builds on that practical starting point. It has not undergone a formal clinical validation or outcomes study.
